As you may know, due to the current health crisis, the Indiana primary date has been extended to June 2, 2020.  

I applaud the action of the Indiana Election Commission to protect our public health and allow all individuals the ability to vote by mail this primary election.  Below are instructions on the steps you need to take to receive your mail-in ballot based on your county

To request a mail-in ballot online, please visit indianavoters.in.gov.

To mail in your application for a mail-in ballot, please complete the following steps below.  If you don’t have a printer, please contact our campaign office and we will send you an application.

1 – Print and complete this form.  Don’t forget under section 3 to check the box for DEMOCRATIC PARTY.

2 – Send it to your county election office by the fax, e-mail, or mailing address below.  The deadline to submit this form to your county office is MAY 21.
